Job Requirements

• At least 1 year of Registered Nurse experience required
• Previous case management and discharge planning experience strongly preferred
• Active RN license in Maryland or compact eligibility
• Strong collaboration and communication skills with patients, families, physicians, and interdisciplinary healthcare teams
• Ability to coordinate transitions of care and support safe, timely discharge planning
• Experience working with acute inpatient populations preferred

Responsibilities

• Coordinate care management activities for the acute inpatient population
• Develop and support appropriate discharge plans in collaboration with patients, families, and the care team
• Partner with physicians, nursing, utilization review, and ancillary departments to promote efficient care progression
• Facilitate transitions of care to the appropriate level of service for safe discharge
• Collaborate with community providers and ambulatory case managers to ensure continuity of care
• Help reduce avoidable readmissions through proactive planning and patient-centered coordination
• Support clinical and cost-effective outcomes through timely communication and resource coordination
