Job Requirements

• Active LCSW license required (Licensed Clinical Social Worker)
• Minimum 1 year of social work experience in a healthcare or hospital setting (inpatient social work preferred)
• Demonstrated experience with care coordination, discharge planning, and care transitions to post-acute care and community resources
• Ability to work collaboratively with interdisciplinary teams (physicians, nurses, case managers, therapy services)
• Strong assessment, documentation, communication, and organizational skills

Responsibilities

• Coordinate patient care and manage discharge planning to ensure safe, timely transitions to home health, skilled nursing, outpatient services, or community resources
• Provide inpatient clinical social work services, psychosocial assessments, crisis intervention, and counseling to patients and families
• Collaborate with interdisciplinary teams to develop and implement individualized discharge plans and improve patient outcomes
• Document assessments, interventions, and discharge plans accurately in the patient record in accordance with hospital policies
• Participate in rotating weekend coverage and support continuity of care across shifts
